Eligibility
Who Qualifies for a Discount?
Your discount is based only on your household size and annual income — nothing else. No asset checks, no credit history, no immigration status. The income thresholds below are based on the 2025 Federal Poverty Guidelines published by the U.S. Department of Health and Human Services.

Getting Started
How to Apply
Applying is simple and confidential. You can apply at any time — before, during, or after your visit. No one will be turned away while an application is being reviewed.
- Request an Application
Ask for a Financial Assistance Application at the front desk or billing office, or call us to have one mailed or emailed.
- Gather Your Documents
You'll need proof of identity (IDNYC accepted), proof of household income (pay stubs, tax return, or written attestation), and proof of household size. If you don't have documents, a signed statement is accepted.
- Submit & Get a Decision
Submit to the billing office. We aim to notify you within 10 business days. Approved discounts are applied to your current and any outstanding balances.
- Payment Plans Available
If a balance remains after your discount, we can arrange an installment payment plan based on your ability to pay.

What You Can Always Expect
No Medicaid Requirement
We will never require you to apply for Medicaid or provide a denial letter to access a sliding fee discount. You do not need insurance to qualify.
No Immigration Questions
Your immigration status is never asked, never recorded for this purpose, and never shared with any government agency. Everyone is welcome.
Income & Family Size Only
Eligibility is based solely on family size and annual income — no asset tests, no credit checks, no payment history reviews.
No Collections During Review
Billing and collection activity is paused while your application is being reviewed. No liens will be placed on your home.
Updated Every January
Income thresholds are updated each year following HHS publication of new Federal Poverty Guidelines. Your application uses the guidelines in effect when you apply.
