- Complete new patient forms (available online or in-office)
- Bring photo ID and insurance card
- Bring a list of current medications
- Arrive 15 minutes early for registration
- Bring prior medical records or imaging CDs if available
What to Expect
At Starling Health, we strive to make every visit as comfortable and efficient as possible.
- Check in at the front desk upon arrival
- A medical assistant will take your vitals and review your history
- Your physician will conduct a thorough examination
- Any needed imaging or testing can often be done on the same visit
- You'll receive a visit summary and follow-up instructions
