Request Your Medical Records

You have the right to access your medical records. Starting a request takes a minute — here is exactly how it works, and what we will need from you before we can release anything.

I need my medical records

Choose whichever is easiest for you. Either way, our staff will contact you to finish the request and have you sign the required authorization form.

The email button opens your own email app with a short form already filled in. Send it to info@starlingdiagnostics.com — or call us and we will take the request over the phone.

Please do not email us any medical details.

Regular email is not a secure way to send health information. Only include the identifying and contact details we ask for — your name, date of birth, phone number, dates of service, the type of records you need, and where to send them. Never send diagnoses, symptoms, test results, medication lists, or images by email. If you need to discuss anything medical, call us at (718) 319-1610 or message your care team securely through the patient portal.

The Process

What Happens After You Ask

Sending a request starts the process — it does not release your records on its own. Federal privacy law requires a signed, written authorization before any medical records leave our office. Here are the four steps.

  1. Send us your request

    Start by emailing us, calling the office, stopping by the front desk, or messaging your care team through the patient portal. This step simply tells us you need your records — it does not release them yet.

  2. Our staff will contact you

    A member of our team will reach out to confirm your identity, make sure we understand exactly which records you need, and tell you where to send them.

  3. Sign the paper authorization

    Before any records leave our office, you must complete and sign a medical records release form. A signed paper authorization is required by HIPAA — this protects your privacy and makes sure records are never released to the wrong person.

  4. We prepare and release your records

    Once we have your signed authorization, we prepare your records and send them where you asked. We honor requests within 30 days, as required by HIPAA.

The patient portal — fastest for recent results

Lab results, imaging reports, and visit summaries are available in the patient portal 24/7. If you only need a recent result, this is usually the quickest route and no paperwork is needed.

By phone

Call (718) 319-1610 and tell us you need your medical records. We will take your information and explain how to complete and sign the required authorization form.

In person

Stop by the front desk at 1480 East Avenue, Bronx, NY 10462 and ask for a medical records release form. Bring your photo ID. You can fill it out and sign it while you are here.

How long does it take?

We honor requests within 30 days, as required by HIPAA. That is the outside limit, not a target. If you need your records by a specific date — for a surgery, a specialist visit, or a school or work deadline — tell our staff and we will do our best to meet it.
